As you can see, we only ever use the vous form.
Note that literally, veuillez would be similar to be willing to, and is therefore used as a polite and formal way to ask someone to do something.
-> Be willing to help me = Please help me = Veuillez m'aider
Not to be confused with the regular conjugation veillez from the verb veiller (to be watchful).
