I dont understand why se faire refaire isn't conjugated to [s'est fait refaire] but s'est fait poser is conjugated.
Also why isn't the causative faire used in the liposuction phrase? :
elle a eu plusieurs liposuccions

1. The sentence is -
Elle a commencé par se faire refaire le nez = She started with having her nose done
commencer par is followed by an infinitive
2. If you used 'faire' instead of 'avoir' you would be talking about what the doctor was doing not the patient having done.
i.e
Elle/il a fait plusieurs liposuccions ce matin = She/he did several liposuctions this morning
